MIXER 5 Interview Sessions

Instructions

To receive compensation for your participation, you must fill out and submit one of the following forms:

  • W9 FORM for US resident tax return filers
  • FORM 8233 for US non-resident tax return filers

If you are a foreign national and have any questions about your eligibility to receive compensation for your participation in this study, please call the IRS at 1-800-829-1040. Ask for the Foreign Issues department.

Completed W9 forms should be mailed or faxed to:

Payment Rates Table

This table gives a breakdown of the rates and bonuses paid for Mixer 5 participation. If you have any questions about compensation rates, please contact us at participant-care@ldc.upenn.edu.

Once you have completed your interview sessions and phone calls, and submitted your W9, you will be processed for payment. Normally, it takes 8 weeks to receive payment.

Target Bonus $140 All 6 interviews and all 10 calls must be completed $140
Foreign Language Calls $2 extra per call Up to 10 calls $20
Foreign Language Bonus $40 4 foreign language calls must be completed $40
Unique Calls $2 extra per call Up to 10 calls $20
Unique Call Bonus $40 4 calls from unique phone lines must be completed $40

Phone Call Disclaimers:


Individuals who participated in Mixer 3 are limited to a
MAXIMUM OF TEN PHONE CALLS for both studies combined. For instance, a participant who completed seven calls in Mixer 3 is eligible to complete three calls in Mixer 5. A participant who completed ten or more calls in Mixer 3 is not eligible to make calls in Mixer 5.

All calls are monitored for duration, language, and signal interference. All calls recorded under a PIN must be from the same speaker. Calls made by different speakers using the same PIN will be disqualified and the owner of that PIN will not receive compensation.
